St. James the Apostle Catholic School is committed to ensuring the safety of our students, faculty & staff, and community members. 

In accordance with the Center for Disease Control (CDC), the American Academy of Pediatrics (AAP), and the Archdiocese of San Antonio, we encourage everyone who is eligible for the COVID-19 vaccine to receive the COVID-19 vaccine. 

If parents would like to voluntarily submit evidence of their child's COVID vaccination card once a child has received both doses, then we invite you to use the form below.  If you choose not to provide such information at this time, you will be asked to submit proof of vaccination status in the event of a close contact situation to avoid quarantine protocols.

You will be asked to enter your child's full name, grade, date of first vaccine, and date of second vaccine. 
Then you will be asked to upload a copy of the vaccination card at the end of the form.

If your child is not vaccinated or you are unable to submit proof of vaccination, then our school will follow the protocols for unvaccinated individuals.  Please see below.

• Close Contact for Unvaccinated with No Symptoms
  • 7-10 Day Quarantine Period Option
• Individuals Previously Diagnosed with COVID-19 within 3 Months Follow Guidelines for Vaccinated Inviduals
• Fully vaccinated individuals are not required to quarantine if not experiencing symptoms
